We are here to serve you and your health care needs.
Services available include:
• Health Screenings (blood pressure and blood sugar)
• Consultation with a nurse practitioner (health and wellness education,
immediate care needs, medication and diagnosis review)
• Care Navigation and Referrals (connection to a medical home, behavioral health referrals, community resource referrals)
